How It Works
We accept Amerigroup/Wellpoint, First Choice Health, Lifewise, Moda, Molina, Premera, Regence, Washington Apple Health. Contact our benefits specialist Laura to confirm your coverage before your first visit.
Out of network? We provide documentation for potential insurance reimbursement.
Book your appointment online or by phone. Once booked, your new patient forms will be available in the Patient Portal.
Request an AppointmentAfter scheduling, you'll receive an email from CHARMPHR to create your account. The portal is where you'll access forms, records, and communicate with our office.
We recommend downloading the CHARM PHR app for easy form completion.
Download the CHARM App
Fill out all new patient forms at least 48 hours before your first appointment. If forms aren't received on time, your appointment will be rescheduled.
All forms are electronic — no need to print anything.
Review consent forms before your visit — they'll be signed electronically. No need to bring any paperwork.
Please arrive on time; if you're more than 10 minutes late, we may need to reschedule. Emergencies may occur and will take priority — we appreciate your understanding.
We value the time set aside for you and your family. We do not double book appointments. If you cannot keep an appointment, we appreciate 48 hours' notice.
